2.When there is a match in progress.
Certain non-steam versions make it possible for a client to enter the game after agstart is called. In this case the client must enter spectator mode or he will be kicked. If there is a second violation by the same client (restart/retry), he gets a permanent ban.

3.Agkick
is allowed, but it is not a toy! If you use it incorrectly, you'll have a month to think about it before being able to join the server again!
